Group Fitness & Gold Club passes
Please fill out this form if you wish to suspend or extend your Group Fitness or Gold club Pass.
Pass suspension requests will be processed within 7 days. Please ensure you have provided at least 7 days notice to request your pass suspension.
- A client may freeze his/her pass once in a 12 month period, for a minimum of 2 (two) week free of charge and a maximum of 4 (four) weeks. The freeze time will be added to the end of your renewal date. During the period that the pass is frozen the member will not be able to use the facilities and related benefits of casual fitness membership.
- In exceptional circumstances, a client may be allowed to defer his/her pass for a longer agreed period (up to a maximum of 6 months), or for more than one occurrence in 12 months. This request must be made to Dovercourt in writing stating the reason for freezing and include supporting medical evidence. Dovercourt reserves the right to decline your application, or apply an administration fee for such a deferral. Any such period deferred will not form any part of the contractual minimum number of payments (if present).
- The above paragraphs a) and b) do not apply to the following pass:
I. Fitness Centre Only Passes. No freezing of this pass is allowed unless accompanied with a valid doctor's note. The minimum is 2 (two) weeks
- A doctor's note also includes Pregnancy where any pass may be frozen until such time as the client is able to return to physical activity.
This form is not a guarantee your pass will be placed on hold; it is a request. You will be notified once your request has been approved.
